Life Membership Application Form

For the VFRS Association to organise a Life Membership badge and complimentary framed certificate on your behalf, the captain and / or secretary of the brigade is required to complete the Life Membership application form.

Please complete the Life Membership application form below and email it to admin@vfrs.asn.au.

Please note: It can take up to six weeks to handcraft the sterling silver Life Membership badge with the recipient and brigade names engraved on it.

Once the application form has been received, the VFRS Association will email an invoice to you for $150. Please notify the VFRS Association once the invoice has been paid. The application will then be processed.
